Interprofessional collaboration in initial-phase intensive support team for dementia in Kobe City

Initial-phase intensive support team for dementia in Kobe City was launched in 2013. The Support Team provides the outreach services to support people with dementia and people suspected having dementia, and their caregivers. The Support Team consists of the medical and welfare specialists such as medical doctors, public health nurses, social workers, and occupational therapists. These professionals cooperate with each other to support community-dwelling people with dementia or people suspected having dementia. We report here activities of the Support Team in Kobe City, and discuss the importance of interprofessional collaboration in community care for support of the people with dementia.
