Community care system of the local government and the Initial-phase Intensive Support Team(IPIST)for dementia

We opened dementia consultation service counter at 27 of Comprehensive Community Support Center in the city of Setagaya from 2006, and an expert counselor of dementia is allocated for each place.
We have been carrying out the measure of consistent dementia program from prevention to care, for example, dementia prevention program, dementia family meeting and home visit nursing for the elderly with dementia.
In 2013, we developed a concept of dementia home life support center, and started the Initial-phase Intensive Support Team(IPIST) for dementia.
To provide early support to dementia people, it is necessary to build not only the IPIST, but various measures which are satisfied their requirements.
