A population-based prospective cohort study of dementia has been conducted in Hisayama town since 1985.@The prevalence of Alzheimerfs disease increased significantly from 1985 to 2012.@In a prospective study of dementia, increased blood pressure variability, diabetes mellitus, albuminuria, and persistent smoking from midlife to late life are significant risk factors for the development of Alzheimerfs disease.@Our findings indicate that improvement and optimal management of these vascular risk factors are important to reduce the risk of Alzheimerfs disease.
