A novel ICT system for interprofessional collaboration supporting home care and treatment of patients with dementia:community approach and trial report

Interprofessional collaboration is critical for home care and treatment of patients with dementia in our community. Although ICT tools have been considered to be suitable for such collaboration, previous systems did not seem to be fully effective. We developed a novel system for interprofessioal collabotration based on ICT （Hikari One Team SP） and evaluated the effectiveness of our system in the community care. In this trial, we found that our system strengthened the interprofessioal collabotration.
