Neuropathology of dementia with Lewy bodies

To investigate the clinicopathological features of Lewy body disease (LBD), a series of 64 subjects with autopsy-proven LBD was studied according to the third report of the DLB consortium diagnostic criteria. According to the 1-year rule, 19 cases (30%) were classified into 1-year rule group and 45 cases (70%) into non-1-year rule group. The pathological diagnosis of diffuse neocortical form was 39%, limbic form 47%, and brainstem form 14%, respectively. The likelihood of occurrence of the DLB was high in 67%, intermediate in 17%, and low in 16%. The pathological involvement of peripheral autonomic system (PAS) showed different patterns in 1-year rule group. Therefore, the investigation of PAS is one of the important factors to assess the LBD pathophysiology.
