症例報告

早期食道胃接合部腺癌によりpulmonary tumor thrombotic microangiopathyおよび癌性リンパ管症をきたした1剖検例

要旨: 症例は70代男性。死亡3〜4週前に食欲不振，労作時呼吸困難，体重減少を主訴に受診した。造影CTで癌性リンパ管症と診断され，精密検査を行ったが原発巣は不明であった。心臓超音波検査で肺高血圧症および右心負荷を指摘された。その後，呼吸不全が急速に進行し，酸素投与開始から6日後に死亡した。剖検ではpulmonary tumor thrombotic microangiopathy（PTTM）および癌性リンパ管症の診断となった。原発巣は10mm大の早期食道胃接合部腺癌であり，組織型は低分化型腺癌であった。さらに，腺癌は両側副腎や腹腔内リンパ節，骨髄に転移していた。自験例を含め，当施設でこれまでに経験したPTTMの3例についても報告する。
