症例報告

多発性骨髄腫の存在を見出すことができたbullous amyloidosis

要旨: 60代前半，男性。1年前から下肢を中心に血疱が出現し，寛解増悪を繰り返したため，左大腿外側から生検が施行された。組織学的に，表皮下水疱の像であるが，水疱直下の真皮に好酸性無構造物の沈着を認め，アミロイドを疑う所見であった。特殊染色，免疫染色を施行し，全身性アミロイドーシスに伴うbullous amyloidosisと診断した。また，精査の結果その背景に，多発性骨髄腫が認められた。HE染色を丹念に観察し，鑑別疾患を挙げていくことが重要である
