TFEB転座型腎細胞癌の1例

要旨: 47歳，男性。右尿管結石を契機に偶発的に左腎腫瘍を指摘された。左腎腫瘍は造影CTで，早期相で濃染し排泄相で低吸収を呈した。ロボット支援腎部分切除術が施行された。摘出腎に径6.4 cmの乳白色を呈する充実性腫瘤を認め，組織学的に淡明細胞型腎細胞癌に類似した領域の他に，好酸性細胞の島状増殖や球状基底膜様物質を認めた。免疫染色で腫瘍細胞はメラノサイトマーカーの他，Cathepsin KやTFEBに陽性であった。さらにFISHでTFEB遺伝子領域の転座が検出された。以上の所見よりTFEB転座型腎細胞癌と診断した。
