盲腸穿孔で発症した，高齢者Epstein Barr virus positive diffuse large B-cell lymphoma, NOSの1例

要旨: 症例は80代，男性。血便，脱力，意識消失のため，救急搬送された。回盲部の潰瘍性病変を認め，待機的に回盲部切除術を施行された。肉眼的には，盲腸に耳介様周堤を有する潰瘍を認めた。組織学的に，潰瘍底には，Hodgkin細胞やReed-Sternberg細胞に類似した大型の異型B細胞が散在しており，CD20，CD30，EBER-ISHが陽性であった。炎症細胞浸潤と壊死を伴い，壊死巣内に取り残された血管の周囲を異型細胞が取り囲む像がみられた。リンパ節にも同様の所見を認めた。胸腹部造影CTで回盲部と腹腔内リンパ節以外に， 特記すべき所見は認めなかった。組織学的所見と臨床像を合わせ， Epstein Barr virus positive diffuse large B-cell lymphoma，not otherwise specifiedと診断した。
