胃型腺癌が発生しgastric adenocarcinoma and proximal polyposis syndrome （GAPPS）が疑われた胃底腺ポリポーシスの1例

要旨: 40代女性。多発する胃底腺ポリープで約10年間経過観察中であったが，一部で腺癌が発生。胃全摘出検体では小弯側を除く胃体部底部にポリポーシスを認め，腺癌やdysplasia，過形成性ポリープを伴った胃底腺ポリポーシスであった。他の消化管にはポリープの発生はなかった。FAPやattenuated FAPも検討したが診断基準を満たさず，家族歴は明らかではないがGAPPSを疑った。GAPPSは近年疾患概念の確立された常染色体優性遺伝の胃ポリポーシス症候群である。文献的考察を加えて報告する。
