症例報告

歯肉原発悪性末梢神経鞘腫瘍の1例

要旨: 60代男性。左上顎歯肉に30mm大の隆起性腫瘤が認められた。組織学的に紡錘形細胞が束状配列し，多核細胞や巨細胞も混在していた。免疫組織化学的にS-100蛋白とNestinが陽性を示し，H3K27me3の発現消失がみられ，歯肉原発Malignant peripheral nerve sheath tumor （MPNST）と診断した。口腔領域MPNSTは非常に稀で，Spindle cell carcinomaとの鑑別において，診断が困難なことが多い。NestinとH3K27me3が特に有用である。
