Low grade dysplasiaを伴う胃底腺ポリープの1例

要旨: 60代男性。検診で胃ポリープを指摘され，内視鏡的粘膜下層剥離術を施行された。肉眼的に15mm大の平坦隆起性病変で，組織学的にlow grade dysplasiaを伴う胃底腺ポリープの像であった。Dysplasiaを伴う胃底腺ポリープは家族性腺腫性ポリポーシス等の遺伝性疾患に多くみられる。偶発的に発見された症例では遺伝性疾患併存の可能性は低い。局所治療後に既往歴や家族歴を踏まえ経過観察することが大切と考えられる。また胃底腺ポリープの診断においては，PPI関連病変との鑑別が肝要である。
