心不全症状が先行した遅発型ポンペ病の1剖検例

要旨: 症例は30代女性。小児期より運動後の筋肉痛と赤色尿を呈し，血中クレアチニンキナーゼ高値を指摘された。死亡約2年前に拡張型心筋症と診断され，肺炎を契機に心不全に陥り，多臓器不全にて死亡した。血中酸性α-グルコシダーセ活性が低値であり，遅発型ポンペ病と診断された。剖検時，左心室は高度に拡張していた。組織化学的に心筋細胞内にPAS反応陽性でdiastaseで消化される不整形な結晶物を認め，免疫組織化学的にLysosome-associated membrane protein 2陽性を示したが，電顕的にはグリコーゲン顆粒の沈着は乏しかった。
