回腸-盲腸切除を行った腸管型Behçet病の1例

要旨: 発症時67歳男性。主訴は血便。下部内視鏡で上行結腸からS状結腸に中等大の潰瘍が多発し当初大腸Crohn病が疑われた。小びらん部の生検では絨毛の形状や杯細胞の保たれる傾向がみられ，細静脈壁に好中球浸潤を認めた。類上皮細胞肉芽腫はみられなかった。HLA-B51陽性。ステロイド投与により軽快。2年9ケ月後に穿孔を発症したため回腸-回盲部切除を受けた。外陰部潰瘍に加え回腸に多発する典型的な深掘れ潰瘍により，眼症状はなかったが腸管型Behçet病と診断された。
