リンパ節生検でKaposi肉腫と多中心型Castleman 病と診断したAIDSの1例

要旨: 症例は30代の外国人男性。全身リンパ節腫脹を主訴に受診した。リンパ節生検にて，被膜の肥厚と線維性結合織の増加が認められ，同部にはヒトヘルペスウイルス（HHV）-8陽性かつD2-40陽性の紡錘形異型細胞が認められ，Kaposi肉腫と診断した。リンパ濾胞の胚中心は萎縮性で濾胞構造はやや不明瞭になり，リンパ濾胞内には中型∼大型のリンパ球と形質細胞が認められ，濾胞間には形質細胞が多数認められる多中心性Castleman病も併存していた。組織所見を踏まえて行われた検査で後天性免疫不全症候群（AIDS）と診断した。Kaposi肉腫と多中心性Castleman病の両者は免疫不全状態でHHV-8感染によって発生するという共通した原因があり，併存することがある。今後，我が国でも増加する可能性があり留意が必要である。
