緩徐な増大傾向を示した脾臓Sclerosing angiomatoid nodular transformationの1例

要旨: 症例は52歳男性。3年の経過観察中に増大傾向を示す脾臓腫瘤に対し脾臓摘出術施行。病変は40×38×20mm大，赤脾髄中に星芒状に広がる白色瘢痕様組織を認めた。組織学的には豊富な線維性間質を伴う血管腫様結節によって構成され，sclerosing angiomatoid nodular transformation （SANT）と診断した。花筵状線維化や閉塞性静脈炎は認めないものの，CD138，IgG，IgG4陽性形質細胞の著明な出現を認め，IgG4関連疾患との鑑別が問題となった症例である。
