扁平上皮癌および腺癌への分化を示す類基底細胞癌様成分を伴う
浸潤性乳房外パジェット病の1例

要旨: 80代女性に発生した扁平上皮癌および腺癌への分化を示す類基底細胞癌様成分を伴った浸潤性乳房外パジェット病（以下，ExPD）を経験したので報告する。組織学的に，表皮内病変は淡明細胞が孤在性ないしは小胞巣状に見られる典型的なExPDであった。浸潤部には淡明細胞と共に，類基底細胞癌様の成分が目立ち，胞巣内には腺管形成と角化細胞が見られた。表皮内の淡明細胞は粘液染色陽性で，免疫染色で淡明細胞と真皮浸潤細胞は共にGCDFP-15，CK7，MUC5AC，HGM45陽性であった。
