冠動脈周囲膿瘍部に急性冠動脈閉塞を来たし死亡した糖尿病性腎不全の1剖検症例

要旨: 症例はPTCAの既往をもつ70代女性で，糖尿病性腎症のため人工透析を受けていた。化膿性脊椎炎に対する入院治療中，抗菌薬に対する反応は良好であったが，入院5日目に急変死亡した。剖検では，大動脈弁に細菌性疣贅があり，左冠動脈前下行枝の閉塞が認められた。閉塞部には偏心性プラークがあり強い好中球浸潤を認め，内腔は血栓のみにより閉塞していた。冠動脈の傍らには膿瘍があり，膿瘍の炎症波及によりプラークの不安定性が増し血栓が形成されたものと考えられた。
