甲状腺混合性髄様・濾胞細胞癌の1例

要旨: 稀な甲状腺混合性髄様・濾胞細胞癌の1例を経験したので報告する。症例は34歳女性。近医にて甲状腺腫大を指摘され，精査の結果，穿刺吸引細胞診で甲状腺髄様癌と診断されたため甲状腺全摘術が施行された。病理学的に腫瘍は薄い線維性被膜に覆われ，充実状構造と濾胞状構造が混在していた。免疫染色では充実状，濾胞状構造共にcalcitonin，CEA，chromoglanin Aに陽性を示し，濾胞状構造を中心にthyroglobulinにも陽性を示した。C細胞，濾胞上皮細胞両者の性質を併せ持つ腫瘍と考え，混合性髄様・濾胞細胞癌（mixed medullary and follicular cell carcinoma）と診断した。
