皮膚atypical fibrous histiocytomaの1例

要旨: 患者は30代半ばの女性。前腕部に7 mm程度の結節があり，約半年で15 mmとほぼ倍の大きさとなったため来院し，腫瘍全摘術が行われた。異型の強い紡錘形細胞が花むしろ模様あるいは不規則に増殖し，多核巨細胞や奇怪な核の細胞を混じていた。分裂像は9個/10 HPFで異常分裂像もみられた。しかし背景には典型的な皮膚線維腫の像がみられ，免疫組織学的にα-SMAが部分的に陽性であるが，デスミンは陰性であった。組織所見，免疫組織学的所見をあわせ，atypical fibrous histiocytomaと診断した。
