局所再発を来したMetaplastic breast tumor with
a dominant fibromatosis-like phenotypeの1例

要旨: 症例は78歳女性で，3年前に左乳房C領域に腫瘤を自覚し腫瘤摘出術を受けた。腫瘤の大きさは2.0×1.5 cmで当時の組織診断はfibrous lesion of the breastであった。今回同部位に約2 cmの腫瘤が出現し再び腫瘤摘出術を受けた。腫瘤は初発と同様にfibromatosisに類似した低異型度の卵円形細胞と紡錘形細胞からなり，免疫組織化学にて卵円形細胞はcytokeratin (AE1/AE3）とp63が陽性，紡錘形細胞はα-SMA陽性であった。初発の腫瘤を再評価すると，ほぼ同様の組織所見と免疫組織化学染色パターンでありspindle cell metaplastic tumorのlow grade variantとして提唱された metaplastic breast tumor with a dominant fibromatosis-like phenotypeの局所再発と診断した。
