腹腔鏡補助下S状結腸切除後ドレーン挿入部に発症したポートサイトヘルニアの1例

A Case of Heria at the Port Site Following Laparoscopic-Assisted Sigmoidectmy

要旨: 今回われわれは腹腔鏡補助下S状結腸切除後に発症したポートサイトヘルニアの1例経験したので報告する。
　症例は80歳，男性。S状結腸癌にたいし腹腔鏡補助下S状結腸切除を行った。経過良好であったため，経口開始後の術後第5病日に右下腹部ポート部から挿入留置したドレーンを抜去した。翌日，イレウス症状とポート刺入部の皮下に腸管の脱出と思われる膨出現，腹部立位単純写真で腸閉塞の所見を認めた。ポートサイトヘルニア嵌頓と診断。同日，緊急再開腹手術施行した。手術所見はドレーン抜去部ポート刺入部に約5cmの回腸が嵌頓していた。これを用手的に整復したのち，ヘルニア門を閉鎖した。その後は経過良好であった。
　腹腔鏡下手術に伴う特有の合併症としてポートサイトヘルニアの発生にも注意する必要があると思われた。

Abstract: An 80-year-old man underwent laparoscopically assisted sigmoidectomy for sigmoid colon cancer.On the fifth postoperative day after removal of the drainage tube, he started to complain of nausea and abdominal fullness.An abdominal X-ray examination showed intestinal obstruction, and incarceration of the small intestine was found at the port wound site after removal of the drainage tube.We diagnosed the patient as having ileus due to incarceration of a port site hernia.Emergency reoperation was performed, and this revealed a Richter hernia at the site of trocar insertion in the right lower abdominal wall. Direct closure of the trocar insertion site without small-intestinal resection was performed.Port site hernia is a rare complication following laparoscopic surgery, and the present case illustrates the need to be mindful of it.
