腹腔鏡手術における保険点数に関する一考察

Consideration about the Insurance Mark in the Laparoscopic Surgery

要旨: 内視鏡外科手術が我が国に導入されてから20年が過ぎようとしている。本技術は外科分野において革新的な役割を果たし，現在では消化器外科分野をはじめとしてその普及は目覚ましいものがある。この発展に際してこれまでは技術面や医材の開発の面での進歩が主として注目されてきたが比較的安定期に入った今，医療経済や教育などの内視鏡外科手術のもつ本質的な面での問題点が提起されつつある。今回われわれが行った検討では腹腔鏡下手術に与えられている保険点数の設定基準が明らかでなく難易度に応じた設定がなされていない。医材費に関しても手術点数の中に占める割合が多く，この点を考慮しながらの妥当な点数決定が望まれる。また各メーカーもできるだけ低安価な価格提供はもちろんの事，各施設が常に医療経済の面も考慮しながら手術器具の工夫などを行っていかねばならない。

Abstract: 20 years have past since laparoscopic surgery was introduced into our country. This technique plays an innovative role in the field of surgery.
The progress with a technical aspect and the aspect of the development of medical materials were mainly attracted attention on the occasion of this development till now, but the problems with the essential aspect in this field such as economy or the education have not been concerned until now.
We considered the validity of the surgical insurance from the costs of medical materials according to an operation of endoscopic surgery. The setting of reimbursement for the endoscopic surgery in the health insurance system is not clear, and there is no relation between difficulty of operation and reimbursement. So we should continue to stress the improvement of reimbursement in the health insurance system.
In addition, each company of the surgical instruments must make an effort of offering low price instruments as possible. Moreover each surgeon always considers aspects of the medical economy.
