転移性尿管腫瘍の一例

A Case of Metastatic Ureteral Tumor

要旨: 34歳女性。右下腹部痛を主訴に近医受診。腹部CT，逆行性腎盂造影にて右水腎症，右尿管狭窄，腹水が認められ，当院紹介。尿管生検や画像診断にて尿管腫瘍を疑われ，右腎尿管全摘術を施行した。病理検査では，尿管壁は肥厚し，主に尿管壁内に低分化型腺癌が浸潤していた。転移性の腫瘍を疑い，胃内視鏡および生検の結果，低分化型腺癌が確認され，ボールマンIV型の胃癌と診断された。現在，化学療法施行中であるが，最近，左水腎症が認められ，新たな尿管転移が疑われている。尿管狭窄や閉塞による水腎症を認めた場合，鑑別として転移性尿管腫瘍の可能性を念頭に置く必要があると考えられた。

Abstract: We experienced a case of right hydronephrosis, which was later diagnosed to have caused by gastric cancer. A 34-year-old woman complained of right lower abdominal pain. Computed tomographic scan and retrograde pyelography revealed no signs of urinary stones or tumor except for the presence of right ureter stenosis and hydronephrosis. She admitted for further examination. We suspected a presence of urothelial carcinoma, and conducted right nephroureterectomy. Pathological examination revealed poorly differentiated adenocarcinoma including signet ring cells and vascular invasion in ureter wall. She had no upper abdominal discomfort, but she underwent gastrofiberscopy, which revealed gastric cancer(Borrmann type IV) in cardiac region. The ureteral tumor was the metastasis of gastric cancer pathologically.
