上顎骨に発生したエナメル上皮線維歯牙腫の1例

A Case of Ameloblastic fibro-odontoma in the Maxilla

要旨: エナメル上皮線維歯牙腫は，歯原性上皮と歯原性外胚葉性間葉組織の両者の増殖からなり，象牙質，エナメル質の形成を伴う歯原性腫瘍である。今回われわれは，上顎骨に発生したエナメル上皮線維歯牙腫の1例を経験したので報告する。症例は22歳，男性，左側頬部の腫脹を主訴に来院した。初診時，左側頬部の軽度腫脹を認め，パノラマX線上，極わずかなX線不透過像を含む境界明瞭なX線透過像と埋伏歯を認めた。CTでは，長径約4 cmのsoft tissue density massを認め，内部に小さな石灰化像を伴っていた。良性歯原性腫瘍の臨床診断にて，全身麻酔下に腫瘍摘出術を施行した。腫瘍は周囲から容易に剥離でき，埋伏歯も摘出した。病理組織検査では，細胞成分に富む結合組織の中に歯原性上皮が索状，島状に増殖し，エナメル器に類似した像も認められた。またエナメル質や象牙質の形成も認められ，エナメル上皮線維歯牙腫と診断された。本疾患はまれな歯原性腫瘍で，本邦での過去30年間における報告は，自験例を加え計73例であった。

Abstract: Ameloblastic fibro-odontoma is a odontogenic tumor composed of odontogenic epithelium, odontogenic ectomesenchyme, dentine, and enamel. We report a case of ameloblastic fibro-odontoma in the maxilla. The patient was a 22-year-old man who complained of swelling of the maxilla. On his first visit, slight swelling was found in the left side of the maxilla. A panoramic radiograph revealed a radiolucent image with no distinct radiopaque region and a impacted tooth. Computed tomography showed a 3 cm soft tissue density mass with some small calcified foci. The clinical diagnosis was a benign odontogenic tumor. The lesion was surgically enucleated under general anesthesia. The tumor was easily resected and the impacted tooth was removed. Histologically, the lesion was composed of cellular connective tissue with proliferating odontogenic epithelium in the form of cord and island, and in addition, enamel and dentine were observed. On the basis of these things, the lesion was diagnosed an ameloblastic fibro-odontoma (AFO). AFO is a rare odontogenic tumor, and only 73 Japanese cases, including ours, have been reported for the past 30 years.
