退院サマリについての一考察
―― 退院サマリの質と作成率の向上へむけて ――

要旨: 退院サマリの作成率の低さは病院の種類，大小を問わず，診療録管理部門の大きな悩みの種である。医療法施行規則第二二条の三は，退院サマリを作成することは特定機能病院の義務と定めているが，何故退院サマリを作成しなければならないかという理由や退院サマリを作成する意義の理解なしに貴重な時間を割いてサマリ作成を行わなければならないとしたら，サマリ作成率向上は到底達成できるものではない。本稿ではこれまであまり明確に語られることのなかった退院サマリの存在意義と使用目的を整理した。退院サマリは本質的には『主治医以外の医師，患者，病院管理者，生命保険会社，公的機関に対して，医療の透明性を確保し，行われた医療行為の正当性を簡潔，明瞭に述べる基本資料』であり，『自分以外の関係者のため』に記載されるべきものである。サマリへの必須記載事項は概ねDPCで入力される事項を除けば，入院経過の概要に限られる。入院経過の概要は，自分以外の関係者の診療情報把握を助け，円滑な連携を実現するために記載することを第一の目的として，事実上他に解説を必要としない程度に，平易かつ明快に記載することが求められる。退院サマリの情報は，記載医師にとっても法的価値，診断書類記載上の価値，医学研究上の価値および透明性を保証する説明責任資料としての価値があるので，サマリを適切に作成することは作成医師にとっても十分なメリットがある。さらに退院サマリは，病院にとっては病院統計上の価値，入院診療報告書としての意味合いが，また地域にとっては公衆衛生情報としての価値がある。しかし，これらのサマリの意義を理解したとしても，作成を容易にし，質の高いサマリ管理を行うためには，効率の良いサマリ作成システムや検索エンジンの搭載，作成データの質を保証する十分数の診療情報管理士の確保が必須である。

Abstract: Writing a discharge summary whenever a patient leaves the hospital may seem to be an obvious duty of the doctor in charge. In Japan, however, the significance of discharge summaries is not well understood. A discharge summary is a fundamental document to inform the patient concerned, other doctors and public agencies of the legality and rationality of the medical actions taken during the patient's hospital stay. Discharge summaries are also useful to the doctor in charge, because they help him or her to achieve transparency and accountability. To improve the quality and prevalence of discharge summaries, hospitals should secure an efficient discharge summary writing system, an efficient reference engine and the sufficient number of medical information management specialists.
